An incomplete data set or symptom evaluation can result in less effective communication or even misinformation between multidisciplinary physicians. As a result, unnecessary diagnostic procedures, excessive testing, and complications in treatments plans can occur.
This Symptom Intake and Assessment Tracker provides a detailed compilation of a patient’s symptoms in which data can be entered by the patient, parent or clinician using an application via a computer or smart device. By storing this new data and the patient’s history into a medical database, an individual and thorough profile of each child can be created and shared among physicians.
